Summary

This study analyzed the use of Impulse Oscillometry (IOS) to differentiate and evaluate chronic obstructive pulmonary disease (COPD) and small airway disease (SAD). Researchers examined 306 patients to assess correlations between IOS results, pulmonary function tests (PFTs), and radiological evaluations. IOS was found to be useful in early detection of COPD and SAD, and a novel severity metric, the IOS severity index (IOSsi), was developed to enhance predictive capabilities. An IOSsi score of 4 or higher was associated with increased risk of SAD and COPD, as well as a greater likelihood of acute exacerbations in COPD patients. Additionally, AView COPD software from Coreline Soft was used for quantitative CT analysis, supporting the reliability of IOS findings by assessing emphysema severity and airway wall thickness. The study concluded that IOS effectively differentiates COPD and SAD and serves as a valuable tool for disease monitoring and prognosis
