M. Balbi et al. evaluated the performance of the RISP (Italian Lung Screening Network) program, leveraging web-based tools and AVIEW for automated LDCT analysis to enhance recruitment and screening efficiency. The study enrolled 20,704 volunteers, 10,582 of whom met LDCT eligibility criteria, with 8,747 undergoing baseline scans. AI analysis demonstrated 80% agreement with radiologists in LungRADS classification and provided consistent risk quantification across LDCT scanners. Over a median 12-month follow-up, 91 lung cancers were detected (1.0%), with 55% at Stage I and 77% resected. High recruitment rates (51% eligibility) and early detection underscore the feasibility and potential scalability of a national web-based lung cancer screening network. AVIEW's role in improving diagnostic consistency and optimizing resource allocation was highlighted. Future research should focus on cost-effectiveness and further evaluation of LDCT scanner performance to refine implementation strategies
