Summary

This study investigates the use of a one-piece autologous rib cartilage graft in treating complete naso-orbital-ethmoid (NOE) fractures with hollow space defects. A 57-year-old man with a complete NOE fracture, including loss of bone fragments in the orbital rim and maxillary wall, underwent surgery. The right fifth rib was harvested, mimicking the inferomedial orbital rim, and stabilized with plates. Follow-up CT and 3D volumetry using Α-VIEW software (Coreline Soft, Seoul, Korea) were performed at 1, 3, 6, and 12 months. Results showed a well-positioned rib graft and orbital wall, with a rib cartilage absorption rate of 7.84% at 6 months. The study concludes that autologous rib cartilage grafting is an effective option for reconstructing the bony frame in complete NOE fractures, especially when the buttress for plating is lost, without concerns of significant bony resorption.
